Crushed Grape is committed to positively impacting our community by providing assistance to local schools, churches, organizations and charities.  We are proud to give back to a number of diverse charities and local non-profits whenever possible.

The following Donation Request Policy has been developed to create consistency and fairness to organizations that ask for our support.  Please be aware that we attempt to fulfill as many requests as possible, but cannot guarantee that every donation request will be met.

All donation requests must be submitted electronically on “The Crushed Grape Donation Request Form” at least 30 days prior to the event.

Due to the volume of requests we receive, we ask that inquiries regarding the status of your request not be made.   All organizations will be notified via email whether requests are accepted or declined.

To apply for a donation, please follow the instructions below:

• Download the donation request form by clicking here. (Right Click > Save Target As...)
• Complete the form.
• Send an email to donations@crushedgrapegr.com with the attachments:
• Your completed donation request form
• Cover Letter
• Proof of 501(c)3 Status from the IRS (Letter of Determination or Form 990)

Once these forms are received, we will review each request within two weeks.

All requests must include the Crushed Grape donation request form, a cover letter, and proof of 501(c)3 status from the IRS sent to donations@crushedgrapegr.com to be considered; we can no longer accept donation requests via fax, mail, or phone.
